Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.</p></div><p><strong>This part-time position offers competitive pay, generous benefits and a supportive work environment.</strong></p><p>As part of the pediatric team, the <strong>Pediatric Care Nurse</strong> will provide skilled nursing assessment, planning, and care to maximize the comfort and health of pediatric patients and families in compliance with organization policies and procedures and applicable laws and regulations.</p><p><span style="text-decoration: underline;"><strong>Schedule:</strong></span></p><p>Tuesday, Wednesday &amp; Thursday; 8:30am - 5:00pm</p><p><span style="text-decoration: underline;"><strong>Location:</strong></span></p><p>Santa Clara County with additional coverage to San Mateo as needed</p><p><em><strong>By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.</strong></em><br><br><strong><u>Essential Duties &amp; Responsibilities include&nbsp;</u></strong>(but are not limited to):</p><ul><li>Assume primary responsibility for pediatric patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions</li><li>As applicable, apply specific criteria for admission and re-certification to hospice care or hospice waiver care to establish appropriate levels of care and patient’s eligibility for children</li><li>Initiate communication with attending physicians, other hospice staff members, facility staff members as applicable, and other agencies as needed to coordinate optimal care and use of resources for the patient/family</li><li>Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions</li></ul><p><strong><u>Education/Experience</u>:</strong></p><ul><li style="font-weight: bold;"><strong>One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing</strong></li><li style="font-weight: bold;"><strong>One year of pediatric nursing experience</strong></li><li>Experience in acute care/ home health and/or hospice setting preferred</li></ul><p><strong><u>Certificates, Licenses, Registrations:</u></strong></p><ul><li style="font-weight: bold;"><strong>Current license as a registered professional nurse in the State of California</strong></li><li>Public Health Nursing certification highly desirable</li><li>HPNA certification preferred</li><li style="font-weight: bold;"><strong>ELNEC Pediatric training preferred and required within the first year of employment</strong></li></ul><p><u><strong>Pay Range:</strong></u><br>$58.00 - $72.00 hourly&nbsp;</p><div class="content-conclusion"><p>By the Bay Health is proud to serve our community as an equal employment opportunity employer.&nbsp; Everyone is valued and welcome at BTBH.
