Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.</p></div><p> </p><p>We are currently seeking a <strong>Registered Nurse</strong> to serve as a <strong>Home Health Nurse Case Manager</strong>. The nurse is responsible for providing in-home care for patients, plans and directs services to meet the needs of individuals and families within their home-living environment. The nurse will provide skilled care, education and training to the patient/family based on identified needs to maximize patient rehabilitation and remain safely in the home.</p><p><span style="text-decoration: underline;"><strong>Schedule:</strong></span></p><p>Saturday & Sunday; 8:30am - 5:00pm</p><p><span style="text-decoration: underline;"><strong>Location: </strong></span></p><p>Sonoma & Marin County</p><p><strong><em>By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.</em><br><br><u>Essential Duties & Responsibilities include </u><u>(but are not limited to)</u>:</strong></p><p> </p><ul><li>The RN case manager assumes responsibility to coordinate patient care for an assigned caseload.</li><li>Completes an initial comprehensive assessment of patient to determine home care skilled needs.</li><li>Initiates the plan of care and necessary revisions including obtaining written physician orders for changes.</li><li>Provides direct patient care and services in accordance with the plan of care.</li></ul><p> </p><p><strong><u>Education/Experience</u>:</strong></p><ul><li>BSN degree preferred.</li><li>Minimum of 1 year experience as a Registered Nurse</li></ul><p><strong><u>Certificates, Licenses, Registrations:</u></strong></p><ul><li>Current license as a Registered Nurse in the State of California.</li><li>Current license as a Public Health Nurse in the State of California is highly desirable.</li><li>Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.</li></ul><p><strong><u>Pay range:</u><br></strong>$65.00 - $75.00 hourly</p><p> </p><p> </p><div class="content-conclusion"><p>By the Bay Health is proud to serve our community as an equal employment opportunity employer.
