Remote Transfer Center/Triage RN

In this role, you will provide professional nursing care through telephonic interventions, focusing on the holistic needs of both patients and caregivers. You will manage patient needs through various services including readmission management, nurse triage, and telehealth monitoring. Your commitment to quality standards and safety protocols will ensure effective coordination of patient care and smooth transfer processes. Working remotely, you will collaborate with healthcare teams to achieve patient care goals.

• Assess patient and caller needs through telephonic interactions for readmission management and service referrals.
• Provide telehealth monitoring to evaluate patient conditions and follow up on care plans.
• Facilitate external patient transfers and bed placements for assigned facilities.
• Develop and implement individualized plans of care based on patient assessments.
• Coordinate issue resolution for patients, ensuring timely responses to concerns.
• Document all interactions accurately within established guidelines.
• Crosstrain in various roles within the call center for backup assistance.
• Build collaborative relationships with healthcare teams.
• Participate in case discussions and team meetings to enhance care coordination.
• Adhere to HIPAA regulations.

• Associate degree in multidisciplinary nursing.
• 5 years of experience in nursing, preferably in case management or home care.
• Current RN State License.
