Under the direction of the department manager, the Oncology Certified Nurse will be responsible for helping to develop best practices for managing complex Adult Oncology patients while acting as the content matter expert to assist with plan development and implementation for these patients. The Oncology Certified Nurse will be part of the Innovation team, helping to develop educational content and train staff.

Requirements
Responsibilities:
- Plan and conduct continuing education programs about Oncology conditions and treatment plans
- Participate and support departmental quality improvement activities and projects related to Cancer
- Act as an Oncology related resource for the clinical team
- Provide education and follow up with newly diagnosed and established adults with Cancer
- Conduct telephonic assessment of patients and develop a personalized plan of care addressing all aspects of Oncology self-management
- Identify and address patient concerns
- Collaborate with providers and vendors to ensure patient is compliant with treatment plan
- Identify and secure resources to meet the needs of the patient including financial, psychosocial, physical, and clinical resources.
- Foster relationships within the team to ensure the CareHarmony culture is positive and supportive.
Additional Requirements:
- Registered Nurse (RN) or higher license in compact state required
- Clinical license must be active and in good standing
- A minimum of five (5) years of professional experience in Oncology
- Oncology Clinical Nurse Specialist preferred, but Oncology Certified Nurse (OCN) considered
- Experience in clinical training and process development preferred.
