• Conduct thorough assessments of diabetes patients through virtual consultations, including diagnostics and treatment plan development.
• Formulate individualized treatment plans encompassing medication, lifestyle modifications, and monitoring.
• Collaborate with healthcare team members to provide holistic, integrated care.
• Offer clinical support to clinic staff and assist in medical management discussions.
• Maintain up-to-date patient records while adhering to legal and ethical guidelines.

• APRN with 5+ years in outpatient clinical practice, ideally in diabetes management.
• Minimum of 2 years focused on diabetes education and management.
• Proficient in telehealth technology and electronic health records (EHR).
• Experience in virtual care and a commitment to bringing care directly to patients.
• Bilingual in English and Spanish preferred.
