Accountabilities
  • Conduct thorough assessments of patients' needs, including physical, emotional, and financial aspects.
  • Utilize claims processing tools to develop a member's clinical picture and identify suitable programs.
  • Create treatment plans for standard and complex cases in collaboration with patients and healthcare providers.
  • Monitor treatment effectiveness and adjust plans as needed while documenting measurable outcomes.
  • Maintain ongoing communication with patients, families, and multidisciplinary teams throughout care.
  • Advocate for patients to ensure quality care delivery and assist in overall cost reduction.
  • Achieve productivity and quality standards on a regular basis.
  • Implement cost management strategies and report on outcomes during case management reviews.
  • Build relationships with healthcare providers and clients to facilitate emotional support and guidance.
  • Evaluate wellness program referrals and maintain detailed patient documentation.


Requirements
  • Must hold a current RN license and have graduated from an accredited nursing program.
  • Five years of medical/surgical or acute care experience, including two years in case management.
  • Preferred experience in case management or critical clinical care.
  • Ability to critically analyze medical claims data and develop treatment plans.
  • Strong independent decision-making and problem-solving skills.
  • Knowledge of community resources and funding programs.
  • Proficient in Microsoft Office Suite and other relevant software.
  • Excellent communication, negotiation, and interpersonal skills.
  • Strong customer service orientation and time management abilities.
