POSITION OVERVIEW

We are seeking a dedicated and licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. Reporting to the Director of Rehabilitation, the PTA will deliver individualized physical therapy services to long-term care residents and skilled nursing patients, supporting functional recovery and quality of life under the supervision of a licensed Physical Therapist.

KEY RESPONSIBILITIES

  • Implement individualized physical therapy treatment plans as established and directed by the supervising Physical Therapist
  • Deliver therapeutic interventions including therapeutic exercise, gait training, balance activities, transfer training, and modality application
  • Monitor and document patient progress, functional status, and response to treatment in compliance with state, federal, and facility standards
  • Communicate regularly with the supervising PT, interdisciplinary team, and nursing staff regarding resident goals and progress
  • Educate patients, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques
  • Participate in care planning meetings, utilization reviews, and MDS documentation as applicable
  • Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and facility requirements
  • Uphold resident rights, dignity, and confidentiality in all interactions
  • Assist in maintaining a clean, organized, and safe therapy gym and treatment environment

Requirements

QUALIFICATIONS

Required:

  • Associate's degree from an accredited PTA program
  • Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to start date)
  • Current CPR/BLS certification
  • 1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation setting
  • Experience with geriatric populations and fall prevention programs

Preferred:

  • Familiarity with MDS/PDPM documentation frameworks
