This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for an Intake Assessment Representative, LVN based in the United States.

This role plays a critical frontline function within a psychiatric intake environment, where you will assess incoming patients, support triage decisions, and help determine the appropriate level of care based on clinical urgency and behavioral health needs. You will be responsible for conducting mental status screenings and supporting involuntary hold procedures when required, ensuring patients are directed to safe and appropriate care pathways. The position operates in a high-acuity, fast-paced mental health setting where sound judgment, clinical awareness, and emotional resilience are essential. You will work closely with interdisciplinary teams to support crisis intervention and patient placement decisions. The environment requires strong communication skills and the ability to remain composed in emotionally challenging situations. This is a meaningful role for a licensed vocational nurse passionate about behavioral health and crisis care. Accountabilities:

Conduct initial psychiatric intake assessments and triage patients based on clinical severity, ensuring appropriate placement within the behavioral health care continuum.

  • Perform comprehensive mental status evaluations for patients requiring psychiatric assessment, including crisis and 5150 evaluations when applicable.
  • Support and implement involuntary hold procedures in accordance with applicable regulations and clinical guidelines.
  • Collaborate with clinical teams to determine appropriate levels of care and facilitate timely patient placement.
  • Monitor patient presentations, identify risk factors, and escalate urgent cases requiring immediate intervention.
  • Maintain accurate and detailed clinical documentation related to intake assessments, screenings, and care decisions.
  • Engage with patients and families in high-stress situations with professionalism, empathy, and clinical soundness.

Requirements:

Active LVN (Licensed Vocational Nurse) license and Basic Life Support (BLS) certification are required, along with foundational education in nursing, psychology, social work, or a related field.

  • 1+ year of experience in psychiatric intake, crisis intervention, utilization management, or inpatient mental health care strongly preferred.
  • Strong understanding of psychiatric conditions across child, adolescent, adult, geriatric, and substance use populations.
  • Familiarity with involuntary treatment laws, including California mental health regulations and 5150 processes.
  • Ability to conduct mental status examinations and apply clinical judgment in high-pressure or volatile situations.
  • Strong communication, interpersonal, and documentation skills with high attention to detail.
  • Comfortable working in fast-paced environments involving emotionally distressed patients and families.
  • Basic computer proficiency and ability to interpret insurance or authorization-related requirements.
