Our accredited Diabetes Self-Management Education and Support (DSMES) program—aligned with ADA and ADCES standards—delivers personalized care through live telehealth sessions led by experienced Certified Diabetes Care and Education Specialists (CDCESs) and Nurse Practitioners.

Position Overview
The Nurse Practitioner or Physician Assistant plays an essential role in delivering high quality care for patients living with diabetes and other chronic conditions through telehealth services. The clinician will assess patient health status, review clinical data, provide medication management when appropriate, and collaborate with the care team to support improved patient outcomes. This role involves working closely with the Lead Nurse Practitioner and the Quality Coordinator to ensure that patient care aligns with national standards and best practices. Candidates may reside anywhere in the United States but must hold an active license in the specified state.

Job Duties and Responsibilities
- Assessing diabetes-specific health status
- Reviewing patient medical records
- Conducting monthly 30 minute patient phone visits
- Discussing CGM and/or glucometer data and patterns
- Each NP that prescribes controlled substances is required to have a valid DEA Registration Number.
- Submitting medication refills, prescribing new medication (as needed), adjusting medication and insulin as needed guided by the ADA Standards of Care guidelines and coordination of patient care with care team
- Knowledge of insulin pump therapy, including starting and managing pumps
- Participates in chart reviews with standards, regulations, policies and procedures.
- Provides diabetes education and medication instruction to patients and families while supporting patients in managing their condition and understanding their care goals.
- Attend periodic care team meetings
Qualifications
- Bachelor's Degree required
- Fluency in both English and Spanish required
- Active, unrestricted Nurse Practitioner licensure is required in the state(s) where patients are physically located at the time of the telehealth visit.
- Minimum of 2 years of clinical experience as a Nurse Practitioner or Physician Assistant required
- Experience managing patients with diabetes strongly preferred.
Licensure
- For Nurse Practitioners: Active and unrestricted RN license and APRN license in the State of California
- For Physician Assistants: Active and unrestricted Physician Assistant license in the State of California
Certification
- Current national board certification as a Nurse Practitioner or Physician Assistant through a nationally recognized certifying organization such as AANP, ANCC, or NCCPA
- Active DEA registration required if prescribing medications
- Additional state licenses are a plus
Preferred Experience
- Clinical experience managing patients with diabetes
- Knowledge of diabetes medications, insulin therapies, and treatment guidelines
- Experience interpreting continuous glucose monitoring data
- Familiarity with insulin pumps and continuous glucose monitoring systems
- Certified Diabetes Care and Education Specialist credential preferred
- Previous telehealth experience preferred

- Professional liability malpractice insurance coverage that meets company requirements or willingness to obtain coverage
- Willingness to pursue additional state licensure as required to support patient needs and coverage requirements.
- Ability to obtain credentialing with insurance payers if required
- Must comply with all HIPAA and patient privacy regulations
- Must maintain active licensure and certification throughout employment
- Candidates must be authorized to work in the United States
